Policies

Upon arrival of your first office visit, you will be asked to complete a patient registration and medical history form. Our staff will review your insurance information with you. If you are responsive for a co-payment or we do not participate in your insurance plan, full payment will be required at the time of treatment. We offer assistance in completing insurance forms to facilitate your reimbursement from your insurance company if necessary.
